چکیده

Certain HLA haplotypes were found guilty for certain diseases, and aplastic anemia (AA) is reported to be one of them, especially for HLA-DR2 However, HLA-DR2 frequency in AA reached significance in only one study that contained a racially heterogeneous group. We retrospectively analyzed the results of HLA-DR typing in 26 Turkish AA patients who received either immunosuppressive therapy (IST) or HLA-matched sibling bone marrow transplantation (BMT). Nineteen were male and seven were female. Median age was 22 (range, 12 to 33). In the AA group, HLA-DR2 and -DR3 frequencies were 42.3% and 11.5%, respectively. Other HLA-DR frequencies are given in Table 1. HLA-DR2 positivity was found significantly higher in patients with AA when compared with an unrelated healthy control group ( P = .02). All the patients and the control group (n = 230) were white. The relative risk of AA in DR2+ versus DR2patients is 1.99 (with a 95% confidence interval of 1.19 to 3.32). Eighteen patients received IST and 8 received BMT. Of the 18 patients given IST, 44% (8 patients) had either a complete remission (CR) (6 patients) or partial remission (PR) (2 patients). In DR2+ and DR2patients, overall response rates were 66% (4 CR

Increased expression of HLA DR2 in acquired aplastic anemia and its impact on response to immunosuppressive therapy.

OBJECTIVE To study the frequency of HLA DR2 status of patients with aplastic anemia and their response to immunosuppressive therapy at a tertiary care hospital. METHODS Thirty eight consecutive patients of acquired aplastic anemia were evaluated with respect to demographic features, severity of HLA DR2 status and response outcome to immunosuppressive therapy. RESULTS The mean age of the pat...

An increased HLA DR2 frequency is seen in aplastic anemia patients.

The underlying etiology of aplastic anemia is unknown in the majority of patients, although medications, chemical exposure, or viral infections can be implicated in some. Genetic susceptibility to a variety of diseases has been shown and it has recently been suggested that aplastic anemia is more common in individuals who are HLA DR2+ than in the general population. To examine this question, we...

HLA-DR15 (DR2) is overrepresented in myelodysplastic syndrome and aplastic anemia and predicts a response to immunosuppression in myelodysplastic syndrome.

The extent and importance of autoimmune mechanisms in myelodysplastic syndrome (MDS) and the role of immunosuppression in the treatment of this disease are not well defined. We report overrepresentation of HLA-DR2 and its serologic split HLA-DR15 in both MDS and aplastic anemia (AA). Four clinically and ethnically defined patient groups were analyzed. The HLA-DR15 antigen frequencies among Nort...

Identification of a specific HLA class II haplotype strongly associated with susceptibility to cyclosporine-dependent aplastic anemia.

Hematopoietic function of some aplastic anemia (AA) patients is dependent on the administration of cyclosporine (CyA). To investigate whether certain HLA class II genes are associated with susceptibility to such CyA-dependent AA, we determined the HLA class II alleles of 59 AA patients treated with CyA. Among 26 patients successfully treated with CyA, 13 required a small dose of CyA to maintain...

Increased frequency of HLA-DR2 in patients with paroxysmal nocturnal hemoglobinuria and the PNH/aplastic anemia syndrome.

Many autoimmune diseases are associated with HLA alleles, and such a relationship also has been reported for aplastic anemia (AA). AA and paroxysmal nocturnal hemoglobinuria (PNH) are related clinically, and glycophosphoinositol (GPI)-anchored protein (AP)-deficient cells can be found in many patients with AA. The hypothesis was considered that expansion of a PNH clone may be a marker of immune...
